Medical Release Form
In order for us to communicate effectively with your students physician or outside therapist, as well as completing paperwork such as Vanderbilt Forms or other assessments, a Release Form must be on file. This form should be submitted to the Guidance Office along with the assessment to be completed at least 7 business days prior to your students appointment. Once finished, the forms will be sent directly to the doctors office specified on the release form.
